The form, a simple, one-page PDF, asks for basic information about the referring physician and patient, as well as the patient’s insurance provider, health diagnosis, medications, and plans of treatment. A section for additional comments is also included.

Job-seekers can also peruse the Centers for Specialty Care Group’s job listings, which are regularly updated and span the network's various nursing and rehab centers in New York. Each listing includes application requirements and a brief description of the position.

The Centers for Specialty Care Group’s new Request a Call service allows interested parties to enter their names and contact information, in addition to specifying the NYC nursing home or rehab center they think would best suit them. A Centers for Specialty Care Group representative will follow up with a phone call to discuss the company's offerings in greater detail.
